The ActionHealthNYC program will end its one-year demonstration program on June 30, 2017. Enrollments are not being processed at this time.

All New Yorkers, including ActionHealthNYC participants, can access health care services at hospitals and health centers across NYC.

If you have a regular doctor, you should continue to go to your regular doctor for services.

If you do not have a regular doctor, you can view and download the following lists of low cost health care options in New York City.

You can call any of those locations to seek care regardless of income or immigration status.

In the case of an emergency, any hospital in New York City will care for you, regardless of your immigration status or ability to pay. You will not be turned away.

Health Insurance

Sign up for low or no-cost health insurance and get your doctor visits and prescriptions covered. For more information, visit Get Covered NYC.

ActionHealthNYC can help you receive low-cost, coordinated health care, like:

  • Regular check-ups and shots
  • Screenings and treatment for HIV, tuberculosis, diabetes, blood pressure, cancer and other health conditions
  • Mental health and substance use services
  • Family planning
  • Dental care
  • Services for health problems you have now
  • Help navigating the health care system

With ActionHealthNYC, you choose a Primary Care Home where you go for your doctor’s appointments. At your Primary Care Home, doctors, nurses and other staff will listen to your medical history and help with health care needs.

Your Primary Care Home will help you:

  • Have a regular doctor
  • Find services with other doctors if needed, such as for heart problems or diabetes
  • Answer questions or address any health concerns

ActionHealthNYC members can get access to a network of community health centers and hospitals for primary and specialty health care.


ActionHealthNYC is for New York City residents who have a low income but do not qualify for low-cost or no-cost health insurance in New York through the Affordable Care Act.

You can be eligible for ActionHealthNYC if you:

  • Are a New York City resident
  • Are at least 19 years old
  • Have a low-income (single earning up to $23,540 a year; adults in family of four earning up to $48,500 a year)
  • Are uninsured and not eligible for health insurance through the New York State of Health
