Submitting a Request

The Fire Department of the City of New York commonly provides the public with access to records in compliance with Federal, State and NYC laws. Below are instructions for obtaining commonly requested records by Mail, In Person or Online through Open Records, along with forms that are needed to complete the process. Note all mail requests must include a check or money order for applicable fees payable to "NYC Fire Department" (no cash accepted) and a stamped, self-addressed envelope. Subpoenaing Records

Except as may be authorized by federal or New York State law, all subpoenas for Fire Department records must be "so ordered" by a judge. The so-ordered subpoena, together with the applicable subpoena fee, may be served on the Fire Department in-person in accordance with the requirements of the New York Civil Practice Law and Rules, to:

FOIL Requests

All Freedom of Information Law (FOIL) requests to the New York City Fire Department should be submitted via NYC OpenRecords. Select Fire Department of New York City (FDNY) from the agency dropdown menu.

These records can be obtained by Mail, In Person or Online through Open Records (see dropdown below for instructions). The Fire Incent Report must be obtained prior to requesting the Fire Marshal Investigation Report to make a determination if there is a Fire Marshal Investigation Report.

This record request can only be processed by mail.

Ambulance Call Report/ Pre-Hospital Care Report
These records can be obtained by Mail, In Person or Online through the myPatientEncounters portal.

Please note Ambulance Call Reports/ Pre-Hospital Care Reports cannot be obtained through Open Records.
