The NYPD Candidate Assessment Division has provided below the forms that you must print and prepare prior to reporting for your formal interview with an investigator. Please follow the instructions and complete the forms carefully. If you have any questions on how or when to complete a form, please call the Candidate Relations Desk at 718-312-4226.

First Step

Most NYC government jobs, including the position of New York City police officer, require taking and passing a competitive civil service examination by the Department of Citywide Administrative Services (DCAS).

The NYPD Candidate Assessment Division will only accept forms from candidates who have successfully completed the written Civil Service exam. Upon a candidate's successful completion of the written exam, the department will notify the candidate to proceed with the form application process. The necessary forms can be accessed below.

Online Registration

  1. To open an account, please register here.
  2. Complete the appropriate Personal History Questionnaire online, below.
  • CAS-5 Personal History Questionnaire (for police officers, school safety agents, and cadet applicants only).

Fill out the CAS-5 form online, and follow the prompts. DO NOT MAIL THESE FORMS.
