Before submitting your issue please confirm you have completed these steps:

1. On the Login Page select the Forgot Password link
   
2. Enter the email address or username for your account
   
3. If you use an email address, an email will be sent to that address with a link to reset your password. If you do not see an email from NYC.gov please check your spam folder.
   
4. If you have not confirmed your email address you will be prompted to confirm your email address before being able to reset your password.
   
5. If you use a username you will be asked to provide the answer to the security question you entered when creating your account.
   
6. If you are still unable to reset your password, please submit your issue using this form.
