Mission and Goals
HHC ACO Inc. (the HHC ACO) was formed by HHC as a subsidiary New York not-for-profit corporation on June 8, 2012, to further the goals of the Medicare Shared Savings Program (MSSP) by reducing fragmentation of healthcare delivery, improving care outcomes of Medicare Fee-for-Service beneficiaries and lowering overall growth in healthcare expenditures.
The HHC ACO is specifically designed to deliver seamless, coordinated, high quality care to Medicare Fee-for-Service beneficiaries through an organized group of HHC affiliated physicians, 11 hospitals and other healthcare providers who have agreed to work together to treat a defined population of patients across care settings – including primary and specialty care, hospitalizations and long-term care – and become accountable for the quality, cost and overall care delivered. The HHC ACO focuses on Medicare Fee-for-Service beneficiaries linked to participating primary care providers.
