    Review the Application and Instructions for a Radiation Producing Equipment Permit before applying to determine the requirements and supporting documentation needed to apply for this permit.

      Refer to the Requirements Checklist for basic documentation and other requirements needed to apply for a permit from the Department of Health and Mental Hygiene (DOHMH), some of which depend on the type of business that will be given the permit.

        All outstanding fines imposed for Health Code Violations must be paid before you can apply for or renew a permit issued by the NYC Department of Health and Mental Hygiene. Fines may be paid in person at the Department of Consumer and Worker Protection (DWCP) Citywide Licensing Center or Online.  Click the link below to schedule an in person appointment.

            New and renewal permit applications must be accompanied by proof of coverage for both Workers’ Compensation and Disability Insurance.


            The certificate will include the insurer’s name, policy number, issued and expiration date and must list the DOHMH as the certificate holder (not the policy holder).

            It is important to note that the name and address on the insurance certificates must match that of the applicant or establishment for which an application is being submitted.


            If such coverage is not required, submit a signed Certificate of Attestation of Exemption (Form CE-200) registered with the NYS Workers’ Compensation Board showing the applicant’s assigned exemption number and the date it was registered.

              Proof of a Certificate of Authority to Collect Sales Tax. The address on the Certificate must match that of the applicant or establishment for which an application is being submitted.

                The registration form must be completed by the applicant and must include a complete and accurate listing of all the machines located at the facility.

                  If the facility is a private office, the named applicant must be a Senior Partner.

                  • 9

                    If the applicant is not a private practice office, the applicant is the corporate name of the facility (e.g. the hospital name, nursing home name, etc.). The application must be signed by an authorized representative on behalf of the corporate entity; normally this is the president or chief executive officer.

                      If the facility has a mammography machine or provides mammography services, the application must be accompanied by a copy of the Food and Drug Administration (FDA) certificate.

                        Dental, Podiatric, and Veterinary facilities shall not use X-ray equipment on patients prior to inspection by the Office of Radiological Health or an authorized Certified Radiation Equipment Safety Officer (CRESO). Failure to comply will result in a civil penalty. A list certified CRESOs can be found below.

                          All facilities (except Dental, Podiatric, and Veterinary facilities, which require a CRESO inspection) must work with a medical physicist to submit a report detailing the results of initial quality control tests conducted on all radiation-producing equipment in the facility. In addition, a radiation protection survey shall be conducted for each room housing a radiographic unit. A medical physicist is an individual that has a license to practice the specialty of diagnostic medical physics in New York State, or a current, non-expired CRESO certification in New York State. The initial quality control tests the medical physicist or CRESO will conduct will include all tests mandated by federal, state and local laws to be conducted for the facility type on a daily, weekly, monthly, semiannual, annual and biennial basis.

                              Applying for a DOHMH permit does not satisfy the regulatory requirements of other City, State and Federal Agencies. It is the business owner's responsibility to meet all other requirements. Pursuant to NYC Charter Section 645, no premises may be used or occupied until a Certificate of Occupancy has been issued by the NYC Commissioner of Buildings. Failure to comply with all relevant regulations may result in the suspension of DOHMH permits until proof of compliance is provided.                                 Carefully review all documentation and forms required to register Radiation Producing Equipment with the Department of Health and Mental Hygiene.

                                  When applying online, you will be required to provide information about all radiation producing devices in the establishment. Refer to the Supplemental Forms for specific information you will be required to provide.

                                    Gather all supporting documents required to apply for this permit.

                                      Make individual electronic versions of each supporting document for uploading to the system. The following document types can be uploaded: PDF, JPG, GIF, PNG, XLS, XLSX, DOC, DOCX, VSD, and TXT.

                                        Create an account with the NYC Online Licensing system. Payment is required to complete and submit the application so applicants should have their credit or debit card information readily available. Partially completed applications may be saved and returned later for completion.

                                          Select the type of permit for which you are applying and enter the requested information into each field.  Fields with an asterisk (*) are required.  Applicants will be requested to upload copies of required documents. Please upload each supporting document individually.

                                                In-Person Licensing and Collections Services Available By Appointment Only. DCWP is accepting in-person visits from customers with appointments only at our Manhattan and Queens locations. You must schedule an appointment to meet with a DCWP representative. For health and safety reasons, DCWP cannot serve walk-ins.
