Before you submit your application

    NYS DMV driver's license, non-driver identification card, or the FS-6T receipt. For other acceptable forms of identification, please check Request for Photo Image from DMV under Links to Additional Information above.

      Social Security Card

      • 3

        Two (2) identical passport type color photographs with a white background.

          Current NYSDOL and DEP issued asbestos license(s) and/or certificate(s). The requirements to obtain these licenses and certificates can be found in the following two links:

            An asbestos handler is an individual who is paid to remove, repair, or enclose asbestos-containing materials.

            • 6

              An asbestos handler supervisor is an individual paid to direct and coordinate the removal, repair, or enclosure of asbestos-containing materials.

              • 7

                The Restricted Asbestos Handler Application Fee is $50.

                  Steps to apply in person:

                  Select a course to become a Restricted Asbestos Handler from the list of Accredited NYS Asbestos Training Providers linked under Additional Resources in the About tab. Register for and complete the course. Upon completion of the course, the applicant will receive a NYSDOH Certificate of Asbestos Safety Training for Asbestos Handlers.

                    Download and print the Restricted Asbestos Handler application linked under Additional Resources on the About tab.

                      Complete the application forms, and submit it with all of the documents listed above under Prerequisites, the Certificate issued by the NYSDOH training course, and the required fee, listed below.

                        Mail or hand deliver all materials to the DEP address below under Contact Information.

                        • 5

                          Wait for notification by mail for a time and location to report to for the certification examination. Please call DEP at 718-595-3693 or 718-595-3695 if no confirmation is received within two weeks after submitting the application package.

                          • 6

                            Bring a photo ID card and appointment letter to the certification examination.
