The following documents should be reviewed before hiring staff.
Staff Screening, References, Medicals, Applications and Certifications:
1. Sex Offender Registry by NYS Division of Criminal Justice Services
2. Letters of Reference from most recent employers
3. Staff physical examinations
4. Certifications (i.e. CPR-Pro, Advance First aid, Standard First Aid, Lifeguarding and Lifeguard Management)
All renewing summer camps must upload the documentation below when renewing.
Proof of 501c(3) tax exemption status as well as a letter on letterhead stating your organization;s not-for-profit status is current and in good standing with the US Treasury Department.
Renewals must be submitted online via the NYC Online Licensing system. Registration is required. You will not be permitted to renew a permit if you have outstanding fines. You can pay fines online before renewing.To renew online for the first time online, you will need the Personal Identification Number (PIN) that was mailed to you earlier. The PIN will allow you to link your existing records to one single online account. You only need to use your PIN to link to each permit or license record once.If you need your PIN,you can obtain one by completing an online PIN Request Form or by calling 311.
