Committee Review Protocol

The Community Board's review procedure begins after it receives a 30-day notification form. Please note the SLA requires the 30-day notice be sent by certified mail return receipt requested, overnight mail with tracking number or personal delivery with proof of receipt.

If the notification is for an application for a new on-premises license, or an existing licensee that intends to alter or upgrade its license, Community Board 9 requires separate forms that must be filled out and submitted to the board to be reviewed by the Board's Public Safety Committee. To obtain the Board's SLA Questionnaire and Stipulations forms, please contact the Board Office 718-778-9279 or bk09-1@cb.nyc.gov.
