CHANGES TO RIGGER’S LICENSE

Business Change

When making a change to your business, you must submit the following in person:

  • One (1) 2x2 Passport Size Photograph
  • Department of Buildings Issued License Card
  • If removing a company, a request to remove the company and a search for fines and violations issues to the company by the Licensing Unit. The request can be made by fax, (646)500-6249, or in person.

Note: This must be completed before a company can be removed

  • A notarized letter stating the dissolution of the old company. Dissolution affidavits must be submitted by an accountant on their letterhead, or by and attorney. You will be required to submit the dissolution certificate once the company has been dissolved.  If the company will remain in business, a letter must be provided from the president of the company stating that you will no longer perform rigging work on behalf of this company.
  • A telephone bill with the business name and telephone number on the bill. If you have Voice Over IP (VOIP) you need to submit a Utility Bill or Bank Statement with Company Name at Business Location.

Note: An Example of VOIP would be: Time Warner/Cablevision

  • Insurance Certificates for the new company. Must have the new business address, as well as the business telephone number.
  • If you have Rigging Foremen, you must complete the LIC70 Designation of Rigging/Sign Hanging Foreman Application.  Read the LIC70 instructions
  • If you do not have Rigging Foremen, you must submit a signed, sealed & notarized letter on the company letter head stating so.
  • The appropriate business documentation:

For Corporations

  • Copy of the Certificate of Incorporation, certified by the state
  • Copy of Filing Receipt certified by the state
  • Copy of the Minutes stating the elected officers, including time, date, and location of meeting (applicant must be an officer).
  • Copy of Articles of Agreement. If none, an Affidavit on Corporation Stationery stating there are none, must be submitted.
  • Affidavit with Corporate Name showing Officers of Corporation stating their Names, Titles, Address, and License Number, if any. The Affidavit must contain a paragraph stating that the President of the Corporation will notify the Department of Building within ten days of any changes in the Officers whether they are Licensed or Unlicensed

For LLC’s

  • Copy of Filing Receipt or Certificate of Good Standing, certified by the State
  • Copy of Article of Organization, certified by the State
  • Copy of Operating Agreement (if none, an affidavit on corporation stationary stating such)
  • Copy of the Minutes stating the elected officers, including time, date, and location of meeting (applicant must be an officer).

 

For Applicants whose Business is a PARTNERSHIP

  • Copy of Partnership Agreement listing all Present Officers.
  • Letter on Business Stationery stating Names and Address of all Partners and License Number, if any. The letter must contain a paragraph stating that the Licensed Partner will notify the Department of Building of any changes of Partners whether they are licensed or unlicensed
  • Business Address of Partnership must be in the City of New York. You must submit a copy of a Utility Bill or Bank Statement with Company Name at Business Location.
  • Insurance Certificate for General Liability, Workman’s Compensation, and Disability.


Applicants who are INDIVIDUALS with a business name (Sole-Proprietor)

  • Copy of Business Certificate Certified by the County Clerk
  • Business Address must be in the City of New York. You must submit a copy of a Utility Bill or Bank Statement with Company Name with Business Location.
  • Insurance Certificate for General Liability, Workman’s Compensation, and Disability
  • Fee $50

 

Business Address Change

When making a business address change, you must first submit a LIC33 application (address must be approved before moving to new location) to the Department. Once the Department approves the address, you must submit the following in person to the Department:

  • One (1) 2x2 Passport Size Photograph
  • Original telephone bill with business name and telephone number

NOTE: If your business uses Voice Over Internet Protocol (VOIP) you must submit a utility bill. If ALL utilities are covered as part of your rental agreement, you must provide the rental agreement stating this.

  • Fee $50

 

Business Telephone Number Change

When making a business telephone number change, you must submit the following documents in person to the Department:

  • One (1) 2x2 Passport Size Photograph
  • A notarized letter requesting the change
  • Original telephone bill with business name and telephone number

NOTE: If your business uses Voice Over Internet Protocol (VOIP) you must submit a utility bill. If ALL utilities are covered as part of your rental agreement, you must provide the rental agreement stating this.

Adding an Additional Business to Your License

When adding an additional business to your license, you must submit the following documents in person to the Department:

NOTE: Both businesses must be at the same address

  • Two (2) completed LIC2 applications (One (1) application for each company indicating the other company in the affiliation section)
  • One (1) 2x2 Passport Size Photograph
  • Appropriate business documentation. (See Above)
  • If you do not have Rigging Foremen, you must submit a signed, sealed & notarized letter stating so
  • Original telephone bill with business name and telephone number

 

NOTE: If your business uses Voice Over Internet Protocol (VOIP) you must submit a utility bill. If ALL utilities are covered as part of your rental agreement, you must provide the rental agreement stating this.

  • Department of Buildings Issued License Card
  • Fee $50

Adding Rigging Foremen to Your License

When adding a Rigging Foreman to your license, you must submit the following documents in person to the Department:

  • One (1) 2x2 Passport Size Photograph
  • Original ‘Designated Rigging Foremen’ card for each Foreman.
  • Fee $5 (per Rigging Foreman)

 

Dropping a Foremen from Your License

When dropping a Rigging Foreman from your license, you must submit the following documents in person to the Department:

  • A completed LIC70 Form listing all Rigging Foremen, with the box checked next to the terminated/removed foremen. (Photos of the foremen are not required)
  • The Department of Buildings issued designated Foreman Card (the Licensee should retrieve this card from the Foreman).
