Special Inspection Agency (SIA)
Request to Change Address

Follow these instructions when changing the address of an existing SIA registration.

STEP 1: Complete SIA1 Application

Complete the following sections of the SIA1: Special Inspection Agency Registration form:

  • Section 1 – Mark Change and Address options.

  • Section 2 – Provide SIA registration number.

  • Section 3 – Provide agency information.

  • Section 4 – Provide information for the Primary Director. The Primary Director is required to submit the SIA1 form when adding or removing special inspection categories.  Do not provide information for the Technical Director(s) in Section 5.

  • Section 5 – Skip this section for changing the address of an SIA registration.

  • Section 6 – Skip this section for changing the address of an SIA registration.

  • Section 7 – Skip this section for changing the address of an SIA registration.

  • Section 8 – Provide information for Convictions and Fines

  • Section 9 – Provide name, date, signature and seal for Primary Director indicated in Section 4

STEP 2: Complete Letter Outlining Requested Change(s)

The Cover Sheet (letter) must be provided on agency letterhead and must include a statement that the agency’s address or the Director’s address has changed. The letter must be signed by the Primary Director.

STEP 3: Submit Documentation

Submit the SIA1 application and the letter outlining requested change(s) to:
