NY CONNECTS helps New Yorkers of all ages find long-term care.

NY Connects is a partnership of several community and government-based partner agencies who collaborate to create a system known throughout New York State, as the No Wrong Door (NWD) network. The NY Connects program in New York City is administered by the New York City Department for the Aging and funded by the New York State Office for the Aging. 

NY Connects provides one stop access to free, objective, comprehensive information and assistance to callers. NY Connects is a trusted service to get free information about the range of supports that can help you remain independent in your daily life, whether you want to live in your home, a nursing home, or other residential setting.

Who We Serve

NY Connects serves individuals of all ages and income levels wanting to learn more about long term services and supports in New York City.

NY Connects offers support to:

  • Older adults
  • Children or adults with a disability
  • Family members and caregivers
  • Friends and neighbors

NY Connects staff can:

  • Determine eligibility for benefits and public entitlements
  • Assist individuals in applying for benefits;
  • Connect people to support groups, including those for caregivers
  • Provide referrals and linkages to services like home care
  • Direct individuals to relevant financial and legal information

If you would like to contact your local NY Connects office in New York City, you can find who it is with their contact information below. They can be reached over the phone, via email or by making an appointment. The five organizations listed below are contracted by the NYC Department for the Aging to operate a NY Connects program in the respective borough. Persons who are deaf or hard of hearing may call the NY Relay System 7-1-1.
