Letter Grading for Restaurants

Since 2010, New York City has required restaurants to post letter grades that correspond to scores received from sanitary inspections. An inspection score of 0 to 13 is an A, 14 to 27 points is a B, and 28 or more points is a C. Grade cards must be posted where they can easily be seen by people passing by.

How to Get a Replacement Grade Card

Food Service Operators should print the Replace a Grade Card form (PDF), complete it and have it notarized.

  • The form must be submitted in person at the:

    Bureau of Food Safety and Community Sanitation
    125 Worth Street, Room 1020
    Manhattan
    Hours: Monday to Thursday, 9 a.m. to 4 p.m.; Friday, 9 a.m. to noon


  • Operators must have current government issued-photo identification to receive the replacement card.

  • Mail-in requests are not accepted.
