Perinatal Hepatitis B Prevention Program

  • The program identifies hepatitis B positive pregnant women, conducts case management of infants at risk for perinatal hepatitis B infection & works with hospitals to achieve universal birth dose coverage & documentation of the birth dose in the Citywide Immunization Registry.

Vaccine Information Statements

Under the National Childhood Vaccine Injury Act, all health care providers are required to use Vaccine Information Statements (VIS) to inform parents, guardians, legal representatives, & adult patients about the benefits & risks of vaccines prior to vaccination. You can find a complete list of VISs in many languages at Immunization Action Coalition.

Lifetime Health Records

The Lifetime Health Record can only be ordered by providers giving healthcare to children. To order the Lifetime Health Record, call the Health Department Call Center at (917) 438-9766. Parents who need to replace a lost Record should contact their child's provider.
