Schools, child care centers, camps may now sign up for Online Registry Read-Only access to view immunization records reported by the student's health care provider(s) to the CIR. You must first register your site online and then complet the forms listed below.
To access the Online Registry, the principal/director/owner of the school designates a Security Administrator (User Manager). The Security Administrator (User Manager) must complete the, Security Site Administrator form, page 2- and the Confidentiality Statement, page 3 from the information packets below then mail or fax complete forms back to CIR at (347) 396-2559.

Next, the Online Registry assigns a Username and Password to the Security Administrator (User Manager). The Security Administrator (User Manager) can then set up Usernames and Passwords for additional staff members. He or she must have each additional user read and sign a User Confidentiality Statement (PDF) for Online Access and keep those on file at the site.
