CIR Access

Access to NYC Citywide Immunization Registry (CIR) applications is now via a Single Sign-On (SSO) solution after signing on with individual work email, through the CIR Authentication Server tool. Users can then access multiple applications.

Use the CIR Facility Manager application to:

  • Register your facility or practice with CIR for authorized users; facilities may include:
    • Hospitals, clinics, medical offices
    • Independent retail pharmacies administering immunizations, but that are not affiliated with a medical facility (read/write access)
    • Practices outside NYC (read-only access)
    • Schools, day cares, and camps (read-only access)
    • Foster care agencies, WIC, managed care organizations and other agencies
  • Set up a Site Security Administrator (SSA) Account to manage users at your site
  • Update your registration information. Log in to the CIR Facility Manager, then navigate to the CIR Facility Lookup tab. Do not re-register.
  • Retrieve your Facility Code if you forgot it by logging in to the Facility Manager and then navigating to the CIR Facility Lookup tab. Do not re-register.

Use the Online Registry to:

  • Look up records manually
  • Order and report vaccinations
  • Produce reports
  • Generate populated CH205 form
  • Sign up to participate in other programs (VFC/VFA)

Site Security Administrator

New Registrations: Facilities, Practices

At the time of registration of a facility with the Citywide Immunization Registry each health care facility or practice designates a Site Security Administrator (User Manager). The Site Security Administrator (User Manager):

  • Electronically signs the Security Administrator (User Manager) User ID/Password Request Form, and
  • Agrees to the Acceptable Use Protocol and Security Administrator (User Manager) and Confidentiality Statement for Online Access.

During the approval process, you may be asked to send a letterhead for your facility.

The Site Security Administrator (User Manager) can set up Users for additional facility or practice staff members from inside the Online Registry in the Settings, Manage Users screen. The User Manager must have each additional user read and sign a User Confidentiality Statement (PDF) for Online Access and Acceptable Use Protocol and keep those on file at the site. The Department of Health and Mental Hygiene offers free training for all health care providers on how to gain online access to the Registry. To arrange for training at your office, contact cir-reset@health.nyc.gov or call (347) 396-2400.

For Online Registry Users with Existing Accounts

Starting on October 3, 2023, the Online Registry log-in screen will be replaced with the CIR Authentication Server log-in screen. The CIR Authentication Server Online Registry log-in screen will prompt you to create a single sign-on account to access the CIR Online Registry application. The account will be tied to your individual work email address and a new password that you will create. It is best to update your facility’s information in the Facility Manager application and navigate to the CIR Facility Lookup tab, if you have not done so this year. Continue to access the Online Registry by bookmarking nyc.gov/health/cir, and click on the Online Registry Go icon.
