Records Request

The Fire Department of the City of New York commonly issues records to the public. Below are instructions for obtaining common documents by mail or in-person, along with forms that are needed to complete the process. Note, all mail requests must include a check or money order for applicable fees payable to "NYC Fire Department" (no cash accepted). Include a stamped, self-addressed envelope. FDNY does not process record requests online. For in-person record requests go to:

Subpoenaing Records

Except as may be authorized by federal law, all subpoenas for Fire Department records must be "so ordered" by a judge. The so-ordered subpoena, together with the applicable subpoena fee, may be served on the Fire Department in-person in accordance with the requirements of the New York Civil Practice Law and Rules, to:
