Permit Description

• Open Fires – Diesel or LPG/Propane Fueled - Portable Devices  – Tar Kettle

Permit Length

• Permit not to Exceed a Year
• FD permit expiration will coincide w/ DOB (or Authority having jurisdiction) 

• Work permit expiration date 

Application Requirements

• Letter of request on company Letterhead
• Proof of Liability Insurance
  Department of Buildings (DOB) or Authority having jurisdiction work permit

• Certificates of Fitness  
  S-93 for Diesel fueled Kettles and G-60 for Torch Operator 
  G-40 for Propane/LPG fueled Kettles, 
  F-60 Fire Guard at Construction Sites (must be differentindividual than Torch Operator) 

Additional Information

• Variance Letter from the Fire Department
• An inspection of the site with the Tar Kettle(s) present is required
• Log of F-60 holders hourly inspection and re-inspection ½ hour and 1 hour after operations  cease
