Mortgage Servicing Instructions

Required Documents

In order to process your homeowner mortgage servicing request, you must submit the following:

Mortgage Servicing Request Form:

  • Print and complete a Mortgage Servicing Request Form (PDF) providing ALL requested information. Submission of an incomplete request form will add to the processing time of your request.

Mortgage Servicing Fee:

  • There is a non-refundable $400 fee for ALL servicing requests. Please submit a Certified Bank Check or US Postal Money Order payable to: New York City Department of Finance
  • Be sure to include on the front of the Certified Bank Check or US Postal Money Order:
    • Homeowner Name(s)
    • Address of property
    • Borough, Tax Block and Lot Numbers (lookup)
  • Personal checks and escrow checks are not acceptable and will be returned.

Proof of Residence:

  • In most cases, the NYC mortgage and the deed to a home purchased through an HPD program require that you occupy at least one unit in the home as your primary residence for a specified number of years. Please print and sign an Affidavit (PDF) confirming the residency requirement and submit the notarized original, along with supporting documentation (see below).
  • If you are not required to occupy the home as your primary residence, please submit a brief letter of explanation.
  • If you are required to occupy the home, but no longer reside there, please submit a letter of explanation with proof of residency evidencing the date through which you occupied the property as your primary residence. Proof of primary residence must include a clear, legible copy of at least one (1) document from each of the lists below.

    List 1

    • NYS Photo Driver License
    • NYS Photo Non-Driver ID Card
    • W-2 Form for most recent tax year
    • A Transcript of US Tax Return

    List 2

    • NYS Motor Vehicle Registration
    • Current and mailed US Bank Account Statement (primary mortgage statement is not acceptable)
    • Current and mailed US Credit Card Statement
    • Current and mailed Utility Bill (gas or electric and must include return payment stub)
    • Current and mailed Homeowner Insurance Policy
    • NYS School Registration
    • NYS Voter Registration

Additional Documentation or Action Required

Additional documentation or action is required depending on the type of your request, building, and financing. Please click or press enter on any that apply to your servicing request to reveal requirements.

Refinancing

  • Mortgage Schedule from lender's current title report
  • Commitment letter signed by homeowner
  • Good faith estimate or loan estimate form signed by homeowner
  • Current primary mortgage payoff statement
  • HUD-1 settlement statement from closing of property purchase
  • Preliminary HUD-1 or closing disclosure form for current refinancing and HUD-1 from any previous refinancing (if applicable)
    • If a home equity lien exists, the preliminary HUD-1 Settlement Statement for the current refinance must show that the home equity line will be paid off from the proceeds of the refinance
  • For Homefirst program mortgages only: a copy of the lender's appraisal of the home

Sale or Transfer of Property

  • Mortgage Schedule from Purchaser's current Title Report
  • HUD-1 Settlement Statement from Current Homeowner's Closing of Property Purchase
  • Executed Contract of Sale (for proposed resale)
  • For Assignment and Assumption of Mortgage Only: Notarized Letters from both the current owner and the prospective purchaser confirming the current owner's intent to assign the NYC mortgage to purchaser and purchaser’s intent to assume all obligations and liabilities under the NYC mortgage from the current owner.
  • Other Change in Title to the Home: Homeowners seeking to change the title to their home due to a change in personal circumstances should provide documentation evidencing such change (as applicable, Marriage Certificate, Divorce Decree, Separation Agreement, Death Certificate, etc.).

Payoff and Satisfaction

  • HUD-1 Settlement Statement from Current Homeowner's Closing of Property Purchase

Cooperative Apartments

Additional Required Documents if the Home is a Cooperative Apartment:

  • Certificate of Shares
  • Proprietary Lease
  • Letter from the Management Company stating history of occupancy and subletting
  • Schedule A from the Offering Plan

Senior Citizen Home Assistance Program (SCHAP)

If you received SCHAP financing assistance for the repair of improvement of your home and now seek to sell or otherwise transfer your home or wish to pay the outstanding subsidy balance or otherwise obtain a satisfaction or release of the NYC mortgage or UCC lien on your property, then you must first contact The Parodneck Foundation for assistance.

After The Parodneck Foundation processes your service request and verifies to HPD that you have satisfied all of the obligations of your SCHAP loan, HPD will issue a satisfaction of mortgage or UCC termination, as applicable.

Neighborhood Housing Services (NHS) Programs

If you received NHS financing assistance for the repair of improvement of your home and now seek to sell or otherwise transfer your home or wish to pay the outstanding subsidy balance or otherwise obtain a satisfaction or release of the NYC mortgage or UCC lien on your property, then you must first contact NHS of New York City for assistance.

After NHS processes your service request and verifies to HPD that you have satisfied all of the obligations of your NHS loan, HPD will issue a satisfaction of mortgage or UCC termination, as applicable.

If additional documentation is requested, please submit to mortintake@hpd.nyc.gov. Be sure to include: Homeowner Name(s), Address, Borough, Tax Block and Lot Numbers with your submission.

The failure to submit all required materials may delay or prevent the processing of your request.

The Homeowner Mortgage Servicing unit processes all requests in the order in which they are received. Please do not send duplicate requests, as this will only delay the processing of your application. Please allow several weeks for processing.
