Yes, if you or your authorized representative cannot come to the above HRA locations to apply for D-SNAP PD, please call Infoline at 929-221-0047 to schedule a home visit.
Note: There is no online application for D-SNAP PD. You can print the application before applying, but the only way to apply is at a center or through a home visit.
