• CB 1 Guidelines

Applications are considered first by the Licensing & Permits Committee before being considered by the full Board. Please complete and submit both of the following forms electronically to CB1 seven days prior to the meeting:

• Summary Sheet Form

Please do not submit a 30 day notice unless you are ready to give complete information on the questionnaire and summary sheet by the times noted in the guidelines. The Committee may postpone or decline review if these forms are incomplete or if proper public notice has not been posted within the time noted, and applicant will be asked to write a letter to the SLA and CB1 as described in the Guidelines.

All liquor license applicants must sign and have notarized a stipulations sheet indicating terms of operation agreed upon at Committee. Prior to the meeting please fill out only the top, bolded section and questions 2, 7, 11, 13 and 15 of the following Stipulations Sheet.
