Vaccine Eligibility

I am a person with a disability, do I qualify for the vaccine?

  • Starting February 15th, 2021, individuals with "Underlying Conditions" will be eligible for vaccination. "Underlying Conditions" have been defined as People who have:
    • Cancer (current or in remission, including 9/11-related cancers)
    • Chronic kidney disease
    • Pulmonary disease, including but not limited to COPD (chronic obstructive pulmonary disease), asthma (moderate-to-severe), pulmonary fibrosis, cystic fibrosis and 9/11-related pulmonary diseases
    • Intellectual and developmental disabilities including Down syndrome
    • Heart conditions, including but not limited to heart failure, coronary artery disease, cardiomyopathies or hypertension (high blood pressure)
    • Immunocompromised state (weakened immune system), including but not limited to solid organ transplant or from blood or bone marrow transplant, immune deficiencies, HIV, use of corticosteroids, use of other immune weakening medicines or other causes
    • Severe obesity (body mass index of 40 kg/m2 or higher), obesity (body mass index of between 30 kg/m2 and 40 kg/m2)
    • Pregnancy
    • Sickle cell disease or thalassemia
    • Type 1 or 2 diabetes mellitus
    • Cerebrovascular disease (affects blood vessels and blood supply to the brain)
    • Neurologic conditions including but not limited to Alzheimer's disease or dementia
    • Liver disease

Do I need to provide proof of my medical condition at vaccination sites?

When you schedule your vaccination appointment, you must certify you have an underlying condition that makes you eligible. You will not need a doctor’s note or other proof of your condition to get a vaccine in NYC. You may not be required to certify your condition if you are vaccinated by your usual health care provider.

I don't qualify for the Vaccine but believe I should, who can I contact?

The current eligibility criteria that NYC must follow is being determined on the State level. If you would like to advocate that the eligibility criteria be expanded we recommend you contact the NYS Governor's Office.

Making Appointments

How can I make an appointment?

  • You can make an appointment online using the NYC Vaccine Finder
    Please note that not all Vaccine Sites listed on the Finder are operated by the City. Many of them are privately owned pharmacies and medical centers.

  • You can make an appointment for NYC Health Department and NYC Health and Hospitals run sites by calling 877-VAX-4NYC (877-829-4692)

Can people who do not have access to the internet make an appointment for vaccines over the phone?

Yes, call 877-VAX-4NYC (877-829-4692) for sites that are run by the NYC Health Department and NYC Health and Hospitals. Individual providers outside of this system have their own appointment sign up process, which vary.

Vaccine Accessibility

Where can I find a list of accessible vaccination sites?

On the NYC Vaccine Finder there are accessible sites noted by a physical accessibility icon next to the address.

The omission of a symbol does not mean the site is inaccessible but that the data is not available at the moment. Whenever possible, call ahead for access questions.

How can I report accessibility barriers for Vaccine Sites?

You can report any barriers for Vaccine Sites to the NYC Commission on Human Rights (CCHR) by dialing 311 and say “human rights,” texting 311692, or filling out the CCHR Discrimination Report Form

I can't leave my home, can the Vaccine be delivered to me?

Yes, New York City is providing in-home Covid-19 vaccinations (Johnson & Johnson) to eligible NYC residents who are fully homebound, have not already been vaccinated, and do not already have access to a vaccination program.

New Yorkers who believe they are eligible for an in-home vaccination can express interest using the form below. The City will use this information to call back to further discuss eligibility for the program.

I have a disability that makes it difficult for me to wait in a long line, can I skip the line?

  • For City-run sites, all vaccination is done by appointment so lines should be kept to a minimum. We advise that people arrive no more than 5 minutes before their appointment time.

  • For non City-run sites, there may be some limited waiting space for people needing accommodation. We recommend speaking with the staff on site upon arrival.

What accessible transportation options are available to get to my appointment?

  • If you are 65 and older or have an ambulatory, vision, intellectual or developmental disability and need an ambulette, you can schedule free transport with Hunter ambulettes (516) 812-9827.

  • If you are 65 and older or have an ambulatory, vision, intellectual or developmental disability you can schedule a free ride with Curb at 646-349-0289. If you are a minor under the age of 18, you must have your parent/guardian call to book the trip on your behalf.

  • For individuals who use Curb/Hunter services for their first dose appointment, you will receive a code which can also be used for the second dose appointment. Individuals should call Curb/Hunter directly to schedule transportation for their next appointment.

  • If you have Access-A-Ride you can schedule an appointment by calling 877-337-2017

  • If you have Medicaid-provided transportation, you can use your usual contact number to schedule transport. Alternatively, you can call 844-666-6270.
