If you reported a crime or lost property, you may request a verification report by submitting a Verification of an Incident.

Law Enforcement Certification (USCIS Form I-918 Supplement B)
A law enforcement certification is a federal form that victims must include as part of an application (otherwise known as a petition) for U nonimmigrant status. You must ask a law enforcement agency to complete the form. Please visit US Citizenship and Immigration Services (USCIS) for details, and to download the I-918 Supplement B form.

Stalking Log Form
If you believe you are a victim of stalking, it is important to track the stalking incidents and behavior, especially if you are going to report this behavior to police. Download the Stalking Resource Center's Stalking Incident Log.

Family Offense Petition
To start a proceeding in Family Court, you will need to first file a form called a Family Offense Petition. This form, as well as additional information, can be found at nycourts.gov

Report of Motor Vehicle Accident Form
If you were involved in a collision you may need to complete and submit a "Report of a Motor Vehicle Accident," also referred to as a Civilian Accident Report (form MV-104), to the DMV via U.S. Mail.
