HIP HMO Member ID Cards |
EmblemHealth will mail you your new member ID card approximately 10 days prior to the July 1, 2017 effective date of your new plan. Please begin using your new card beginning July 1, 2017 when claiming benefits, and be sure to destroy your previous card. |
