The Diabetes Prevention Program (DPP) is a lifestyle change program that helps people who are overweight and have prediabetes delay or avoid developing diabetes in the future.
The Diabetes Care PATH Program is available for individuals covered by EmblemHealth that have diabetes or gestational diabetes*. This program offers personalized tools, information and support to coordinate medical care and develop healthy lifestyle habits.
The NYC Health + Hospitals/Bellevue has initiated the Plant-Based Lifestyle Medicine Program, offering adults with chronic disease support in transitioning to a healthy lifestyle that includes a whole-foods and plant-based diet.
*Gestational diabetes is diagnosed when a pregnant woman has high blood sugar levels during pregnancy. It may go away after giving birth but increases the risk of diabetes later in life
