OPA Walk-In Services

The OPA Customer Service Desk is a walk-in service by appointment only.

Services include:

  • Receipt of Ordered Deduction Documents
  • Commuter Benefit Program Inquiries
  • Check Replacement Documents
  • Union Service Unit Documents
  • FICA Refund Consent Forms
  • Beneficiary Services Documents
  • Prior Year Duplicate W-2 Forms (Hudson Yards office only)

Please note: OPA Customer Service representative will not be able to help you with your personal pay or time and leave issues. Please address these issues to your agency Human Resources or timekeeper.
