New Yorkers with chronic illness and older adults need routine care. Make and keep your appointments for routine care. Stay in contact with your health care providers. Tell your providers about changes to your diet.


Your health care provider can help you manage your condition and direct you to nutrition resources, such as:

  • Gods Love We Deliver - nutritious, medically tailored meals for people too sick to shop or cook for themselves (requires doctor’s verification)
  • NYC H&H Plant-Based Lifestyle Medicine Program - nutrition program for any adult who has type 2 diabetes, prediabetes, heart disease, high blood pressure, high cholesterol, or is overweight


If you need a health care provider, there are options near you. You can find a community health center that is taking new patients. Visit nyc.gov/health/covidresources. You can also call 844-NYC-4NYC (844-692-4692) or 311. Don't worry about immigration status or ability to pay. Care is available to all New Yorkers.
